Myocardial Infarction and Ischemia, II: Non–ST Segment Elevation and Non–Q Wave Syndromes

چکیده

82 Myocardial infarction (MI) may be associated with abnormal Q waves associated with the typical progression of ST segment elevation myocardial infarction (STEMI) and T wave inversions described in Chapter 8. In many cases, however, myocardial ischemia (with or without actual infarction) may be limited primarily to the subendocardium (inner layer) of the ventricle, often associated with ST segment depressions, rather than primary ST elevations.

Non St Elevation Acute Coronary Syndrome: Contemporary Management Strategies

The term, acute coronary syndrome (ACS), describes a spectrum of clinical conditions ranging from ST-segment elevation myocardial infarction (STEMI) to non–ST-segment elevation MI (N–STEMI) and unstable angina (UA) (Fig. 119.1) (1). These manifestations of acute myocardial ischemia may revert to the presymptomatic state or evolve into a non-Q wave (also termed nontransmural) MI or to Q wave (or...

ST segment shifts are poor predictors of subsequent Q wave evolution in acute myocardial infarction. A natural history study of early non-Q wave infarction.

Acute ST segment elevation is regarded generally as the sine qua non of evolving Q wave myocardial infarction (MI) because such electrocardiographic (ECG) injury is believed to be a marker of transmural ischemia and a forerunner of transmural necrosis. Alternatively, ST segment depression with or without T wave inversion is viewed as the dominant ECG feature of non-Q wave MI. However, this hypo...

Is ST segment elevation non-Q-wave myocardial infarction after thrombolytic therapy a new clinical entity that requires an invasive management strategy?

The Veterans Affairs Non–Q-Wave Infarction Strategies in Hospital (VANQWISH) trial compared a strategy of early diagnostic coronary angiography (followed by revascularization, if necessary) to a conservative ischemia-guided approach (for spontaneous or inducible ischemia) in 920 patients with acute non–Q-wave myocardial infarction (NQMI) (1). In 1998, the investigators reported that patients as...
